 *** Note: Password protected files. Why password protection? It is not because there is anything secret about the contents of the file. We use a password to ensure members and candidates use the right forms when registering for Ordeal Induction weekends. Member registration forms are password protected. Candidate registration forms are not. The password is the admonition, found in your Order of the Arrow Handbook. If you can't find the password, contact your O/A Chapter Leaders - they can tell you.
 BSA Medical Form
Click for Medical Form #34605 to use for O/A Events. Note: Only COPIES of Part A and C need to be included with your payment for any O/A event. Your COPY wil be returned to you when you check out at the end of the event. Any left over copies of medicals after events are destroyed, in compliance with National BSA Policy.
