BSA Form 680-001 - Medical Form Parts A, B & C (Use this form for all Regional and National O/A events. Mail a COPY of your medical form with your event payment form. Your COPY will NOT be returned to you. It will be destroyed at the end of the event. Note - If you need a physical for school sports, for summer camp and for O/A events: The best time to get a physical exam may be between May 1 and summer camp each year. It will be good for summer, fall, winter and the following spring sports and also valid for summer camp and summer regional & national o/a events. Check the Michigan and Indiana State School Sports websites for current requirements for physical exams for sports. In 2009, school physicals had to be after May 1 for the upcoming year. Take both this form and your school physical exam form with you to the doctor.)
